• If you already received a passcode (a “Custom ID”) after talking with one our study team members on campus, you can click on the following link to enter the survey:

• If you received an email about the study AND you completed
  part or all of the survey last fall, please click on the link provided in the email in order to access the survey.

Once you finish completing the survey online, you can visit us on campus to get your height and weight measured and pick up your $50 Target gift card.
